This case manager will assist consumers in accessing, linking, coordinating,  and monitoring appropriate resources to establish their independence and well-being within the community setting. Case management services are delivered within appropriate environments including but not limited to home, community, educational, vocational, and treatment, utilizing strengths-based approach. </div> <bR> <div>         Qualified candidate  must have at minimum a bachelor's degree in a behavioral heath or other human service related field. Act 33, Act 34 and Federal FBI Clearances will be required.
