This position is responsible for providing case management and advocacy services to a targeted population of adults with serious and persistent mental illness and children with serious mental illness or emotional disorders. This case manager will assist consumers in accessing, linking, coordinating, and monitoring appropriate resources to establish their independence and well-being within the community setting. Case management services are delivered within appropriate environments including but not limited to home, community, educational, vocational, and treatment, utilizing strengths-based approach.
Qualified candidate must have at minimum a bachelor's degree in a behavioral heath or other human service related field. Act 33, Act 34 and Federal FBI Clearances will be required. Competitive salary with excellent fringe benefits provided, commensurate with experience and education of the candidate.
